Check list of documents and information therein to be scrutinised at the time of renewal of licence under MFPO, 1973
(1) FORM-A - Application for renewal, Items 1 to 6 should be filled up properly as in the case of Application (Form "A") for grant of Licence and there should not be any discrepancy. If there is any change, then it should be substantiated ie. when and how it was done? The address should be complete with Pin Code no. and telephone no., if any, has to be given.
Item 7 should indicate the existing licence number and also other licences as indicated at the time of grant of licence.
In Item 8, quantity and value of MFP manufactured during last year should be given. If there was no manufacture during the last three years, licence need not be renewed.
(2) The renewal fee should be paid for five years.
(3) Annual medical examination certificate of workers for the current year.
(4) Wherever the system of issue of a licence by the local authority on annual payment basis, they have to produce a copy of such a licence for the current year and whenever not possible for the previous year.
(5) Any other information like change in partnership-deed, addition to the premises, change in the list of equipment & machinery, addition or deletion in the list of MFP should also be informed to the licensing authority.
